Your childs school is working with the Navajo Nation to evaluate the schools effectiveness and climate. Your responses to the following questions will contribute to that evaluation. Completing the questionnaire should only take about 15 minutes. Under no circumstances will district or school personnel review individual responses. Participation in this survey is voluntary. Some families have more than one child attending a school. Responses may depend upon which child is being referenced. At the bottom of the page is the name of your child as listed by school records. The inclusion of that name is simply provided to ensure clarity for your responses. You may tear the name off the corner of the survey before your return it. Thank you for your cooperation.
